Health insurance or Medicaid said no in Iowa? Deadlines and free help
In Iowa, your health plan is regulated by Iowa Insurance Division (IID), if it is a state-regulated plan.
Iowa runs its own independent review of denials: External Review. You generally have 4 months to ask for it.
External review means a doctor or reviewer outside your plan decides again. Internal appeal means your plan looks at its own decision again.
Pick your plan type
Which kind of coverage was denied? The plan type decides which rules and deadlines apply.
Insurance I bought or got through work
Your state insurance rules may apply. First appeal to your plan, then ask for an outside review.
My employer pays claims itself (self-funded)
Federal rules apply, and most state insurance laws do not. Free help comes from the U.S. Department of Labor.
Iowa Medicaid
You ask the state for a fair hearing. In a Medicaid health plan, you usually appeal to the plan first.
Marketplace (HealthCare.gov) decision
A decision about whether you can buy a plan or get financial help is appealed to the Marketplace.
Checked against official sources: October 30, 2026.

How to tell which agency regulates your plan
Check with your employer which type of health insurance it offers. If the plan is self-insured, questions or complaints go to the U.S. Department of Labor rather than the Iowa Insurance Division. You cannot file a complaint with the Iowa Insurance Division if your policy was issued or bought in another state.
Step 1: Appeal to your plan in Iowa
What it is called: Internal grievance (internal appeal) with your health carrier
If you filed a grievance about a denial and the carrier has not sent a written decision within 30 days (unless you asked for or agreed to a delay), you are treated as having finished the internal process and can ask for external review.
Law: Iowa Code 514J.106(2)
Step 2: Outside review in Iowa
External Review
Deadline to ask: 4 months.
Urgent care in Iowa: faster decisions
Outside review
Expedited review is available when waiting could seriously jeopardize your life or health or your ability to regain maximum function, when the service would be significantly less effective if not started promptly, or when the denial is about an admission, availability of care, continued stay or emergency care and you have not been discharged. Your treating provider must certify the need and may call the Iowa Insurance Division to request it by phone. Call the Division before sending papers. The decision comes within 72 hours.
Law: Iowa Code 514J.106-514J.110, 514J.115; Iowa Code 191 IAC 76
Medicaid
You can ask HHS for an expedited (faster) hearing at any time in a Medicaid appeal. For a denied service after an expedited managed care appeal, the proposed decision is due within 3 working days after HHS gets the plan's case file; for an expedited Medicaid eligibility appeal, within 7 working days after HHS gets the request. HHS tells you quickly whether expedited review is granted.
Law: 441 IAC ch. 2506 (Contested Cases, ARC 0300D, eff. 7/1/2026), 2506.4(6)-(7), 2506.33, 2506.35
Complaints about your plan in Iowa
You can file a complaint with the Iowa Insurance Division against an insurance company or other entity it regulates. You cannot file one if your policy was issued or bought in another state.

How fast a plan must decide a prior approval
For state-regulated health plans, a decision on a prior authorization request is due within 48 hours for urgent requests and within 10 calendar days for non-urgent requests (15 days if the case is complex or the reviewer has an unusually high volume). The provider must get a receipt within 24 hours. Once a service is given under an approval, the approval generally cannot be revoked afterward. This does not apply to Medicaid or Hawki plans.
Law: Iowa Code 514F.8(1)(g), (2)-(3)
Step therapy exceptions
If your plan requires you to try other drugs first, you and your prescriber must have a clear, easy process to ask for an override, and the plan must approve it in listed situations, for example when the required drug is contraindicated or likely to cause harm, or is expected not to work. You can ask in writing for the clinical review criteria for a drug.
Law: Iowa Code 514F.7(2)-(3)

Changes in 2024–2026
Iowa Medicaid community engagement (work) requirements for Iowa Health and Wellness Plan adults take effect December 1, 2026 for people who apply on or after that date, and apply to current members with renewal dates in or after February 2027 (Iowa HHS, IHAWP Community Engagement Requirements). 2025 Iowa Acts, ch. 108 added prior authorization decision deadlines (48 hours urgent, 10 days non-urgent) to Iowa Code 514F.8. Iowa HHS appeal rules in 441 IAC ch. 7 were rescinded effective July 1, 2026 (2026 Iowa Acts, SF 2463); HHS's replacement is 441 IAC ch. 2506, Contested Cases (ARC 0300D, adopted and filed emergency, effective July 1, 2026), which works with the Uniform Rules on Agency Procedure. The old rule's explicit 90-day Medicaid and 120-day managed care deadlines were not carried into ch. 2506, which sets a 30-day default 'unless federal or state law provides otherwise'.
Law: 7 IAC chs. 2500-2506; 441 IAC 7.4(3)
