About Health Appeal Map

Health Appeal Map is a free, plain-English guide for people in the United States whose health insurance or Medicaid said no. For each state it explains who regulates your plan, how to appeal to your plan, how to ask for an outside (external) review, how to ask for a Medicaid fair hearing and keep your benefits, how to appeal a Marketplace decision, and where to get free help. Free tools work out the deadline from the date on your notice and write the letters in your browser.

We are not a law firm, not an insurer, not a health care provider and not a government agency. We do not represent anyone, we do not argue your appeal and we never contact your plan or your state for you. We never tell anyone that their appeal will win or that they qualify: the plan, the reviewer or the hearing officer decides. We link to official sources: state law, federal rules, state agencies and free consumer help.
