External review in Wyoming: deadline, cost and how to ask
Wyoming runs its own independent review of denials: Independent Medical Claim Review (External Review). You generally have 120 days to ask for it.
External review means a doctor or reviewer outside your plan decides again. Internal appeal means your plan looks at its own decision again.
Checked against official sources: October 30, 2026.

Step 2: Outside review in Wyoming
- Program
Independent Medical Claim Review (External Review)
- Who runs it
- Your insurer assigns a state-approved Independent Review Organization; the Wyoming Department of Insurance oversees the process: Insured health plans regulated in Wyoming (most self-funded employer plans are not eligible)
- Deadline to ask
120 days
Send your written request to your insurance company within 120 days after you receive the insurer's final written denial.
- The deadline runs from
The date you receive the insurer's final written denial (the explanation after its internal review)
- Do I have to appeal to my plan first?
You must first finish all levels of your insurer's internal appeal or grievance process. The checklist allows a request without finishing it if the insurer waived that step or your health care professional certified that you need an expedited review.
- Cost
There is a $15 filing fee, paid by check or money order to the Wyoming State Treasurer. It can be waived if your income is at or below the federal poverty guidelines. The insurer pays the full cost of the review itself.

- What can be reviewed
Claims denied because the insurer says the service or treatment is not medically necessary (or on a similar basis).
- Is the decision final?
The decision is binding on you and on the insurer, and you cannot file another external review for the same denied claim. If the reviewer reverses the denial, the insurer must approve the claim and tell you within 5 days.
- How to ask
Fill out Wyoming's official External Review Request Form (your insurer must give it to you with the final denial). Send it, with a duplicate copy, to your insurer at the address it gives, together with a copy of your insurance card, the insurer's final denial letter (or waiver, or your provider's expedited certification) and the $15 fee or fee waiver request. Sign the release of medical records. The insurer assigns an independent review organization and notifies the Department of Insurance.
